Ivermectin is an antiparasitic agent in the avermectin pharmacological class. It binds to parasite-specific glutamate-gated chloride channels, causing paralysis and death, and is primarily used to treat certain helminth infections such as strongyloidiasis and onchocerciasis.
Ivermectin is an oral medication derived from the soil bacterium Streptomyces avermitilis. It belongs to the macrocyclic lactone family (avermectins) and is formulated for systemic use, with topical products available for certain skin conditions under supervision. In parasites, it disrupts nerve signaling, leading to paralysis and death of susceptible organisms.
Therapeutically, ivermectin treats several parasitic infections. Key approved uses include strongyloidiasis and onchocerciasis. It is also employed for certain intestinal roundworm infections and, in some regimens, for scabies, depending on labeling and guidelines. Dosing and duration depend on the infection type, regional recommendations, and patient factors.
Do not take ivermectin if you have a known allergy to it or to any product ingredient. Use during pregnancy or breastfeeding should be limited to situations where potential benefits justify use, with guidance from a clinician.
Dosing in children is weight-based; very small patients require careful clinical assessment and specific dosing. People with significant liver disease should be monitored for adverse effects and may require dose adjustments or alternative therapies.
In some regions, co-infections with high parasite loads (for example loa loa) can trigger inflammatory or neurologic reactions after treatment. If you develop eye symptoms, fever, or confusion after dosing, seek medical attention promptly and discuss travel or exposure history with your clinician.
Very common: dizziness, fatigue, mild rash or itching.
Common: nausea, abdominal pain, diarrhea, headache.
Uncommon: fever, swelling, transient eye irritation, muscle or joint pain.
Rare: severe hypersensitivity reactions, confusion, seizures, or other serious nervous system symptoms; stop use and seek urgent care if these occur.
Ivermectin is a substrate of P-glycoprotein and is metabolized by CYP3A4. Co-administration with strong P-gp inhibitors or CYP3A4 inhibitors can raise ivermectin levels and the risk of adverse effects. Examples include certain antifungals (ketoconazole), macrolide antibiotics (clarithromycin), and protease inhibitors (ritonavir).
Tell your clinician about all medicines you take, including over-the-counter products and herbal supplements. Concomitant use with other antiparasitics or central nervous systemβactive drugs may alter safety or effectiveness and requires medical supervision.
