Gabapentin was developed in the 1990s and approved by the U.S. FDA in 1993 for adjunctive treatment of partial seizures. Today it is widely used for neuropathic pain and other indications. Pharmacologically, it binds to the alpha-2-delta subunit of voltage-gated calcium channels, reducing excitatory neurotransmitter release without activating GABA receptors.
Gabapentin is a structural analogue of GABA, but its primary action is presynaptic modulation of calcium entry. The drug comes in capsules, tablets, and oral solution and is eliminated mainly by the kidneys. Dosing is tailored to the indication and renal function.
Approved indications include adjunctive therapy for partial seizures in adults and children aged 3 years and older, and postherpetic neuralgia in adults. It is widely used for neuropathic pain and related conditions. Some clinics use it for off-label nerve pain when other treatments fail.
Response varies by patient and condition. In practice, gabapentin may reduce pain or stabilize seizures but is not universal. It is sometimes used off-label for other chronic pains and certain anxiety states under supervision.
Administration requires careful titration. A typical start is 300 mg daily, with gradual increases of 300 mg every 1β3 days toward 900β1800 mg daily, in 2β3 divided doses. In older adults or kidney impairment, slower titration and lower doses are used. Always follow the prescribing clinicianβs instructions.
Take gabapentin with or without food. If GI upset occurs, take with meals. Dosing is adjusted for renal function; severe impairment requires dose reductions. If a dose is missed, take it as soon as remembered unless it is almost time for the next dose; do not double up. Store at room temperature, away from moisture, and keep out of reach of children; discard expired product.
Common side effects include dizziness, drowsiness, fatigue, and swelling of the feet or hands. These often occur early and may improve with continued therapy.
Less common but important effects include coordination problems, dry mouth, and mood changes. Seek care for severe reactions, new or worsening depression, thoughts of self-harm, severe rash, trouble breathing, or signs of an allergic reaction.
Consult a clinician for new or worsening symptoms, poor seizure control, or insufficient pain relief. Do not stop gabapentin abruptly; gradual taper is usually required to avoid withdrawal or seizure recurrence.
Discuss use during pregnancy or breastfeeding, and inform providers about kidney disease and other medications to avoid interactions. Seek urgent help for chest pain, severe allergic reactions, or signs of serious behavior changes.
