What Levofloxacin is

Levofloxacin is the levorotatory enantiomer of the fluoroquinolone ofloxacin and exhibits high oral bioavailability (approximately 99%), enabling reliable systemic exposure and usually once-daily dosing. Since regulatory approval in the mid-1990s, it has become one of the most widely used agents in its class. Its bactericidal activity is concentration-dependent and mediated by inhibition of bacterial DNA gyrase and topoisomerase IV, with rapid tissue penetration and broad spectrum against many Gram-positive cocci and Gram-negative bacilli.

Its favorable pharmacokinetics support distribution to lung, urinary, skin, and joint sites, facilitating therapy for selected systemic infections when guided by susceptibility and clinical context.

What it is used for

Indications include acute bacterial sinusitis, community-acquired pneumonia, acute exacerbations of chronic bronchitis, uncomplicated and complicated urinary tract infections, and select skin and soft tissue infections. It is used for intra-abdominal infections in selected regimens and for documented gynecologic infections in adults when local guidelines permit.

Pediatric use is limited; levofloxacin is generally avoided in children and during pregnancy or lactation due to potential musculoskeletal toxicity. In adults, therapy should be tailored to local resistance patterns, disease severity, and patient renal function, with attention to potential drug interactions and adverse effects.

Contraindications and precautions

Contraindications include hypersensitivity to levofloxacin or other quinolones and concurrent use with tizanidine. Fluoroquinolones are typically avoided in pregnancy, lactation, and in children or adolescents due to risks of musculoskeletal injury. Use with caution in elderly patients and those with renal or hepatic impairment; adjust dosing for reduced creatinine clearance. A history of quinolone-associated tendon disorders warrants careful consideration, and therapy should be stopped at the first sign of tendon pain or rupture. Myasthenia gravis may be exacerbated by fluoroquinolones, and neuromuscular weakness can worsen.

Central nervous system effects (anxiety, insomnia, confusion, tremor, seizures) can occur, particularly in older adults or those with CNS disease. Prolonged QT interval may be observed, especially with electrolyte disturbances or concomitant QT‑prolonging medications; ECG monitoring is advised in high-risk patients. Photosensitivity and phototoxic reactions can occur; advise sun protection. Tendon injury risk is increased in older patients and those on systemic corticosteroids. Avoid use during pregnancy and breastfeeding unless no safer alternatives exist and benefits outweigh risks.

Adverse effects and drug interactions

Adverse effects are generally dose dependent. Common: nausea, diarrhea, dyspepsia, headache, dizziness, insomnia. Less common: rash, photosensitivity, vaginitis, abnormal liver tests. Rare: tendinopathy or tendon rupture, peripheral neuropathy, central nervous system disturbances (including confusion or seizures). The tendon risk is higher in older adults and with concomitant corticosteroids; discontinue at first sign of tendon pain.

Drug interactions include absorption reduction with aluminum, magnesium, calcium, iron, zinc, and sucralfate; separate dosing by at least 2 hours before or 4 hours after. Theophylline, caffeine, and certain antidiabetic agents may require monitoring for toxicity or efficacy. Warfarin can be affected; monitor INR. Coadministration with other QT‑prolonging drugs or electrolyte disturbances increases arrhythmia risk. Tizanidine is contraindicated with levofloxacin. Exercise caution with photosensitizing agents and adjust therapy for renal impairment to minimize accumulation and adverse effects.
