Carbidopa/Levodopa is a fixed-dose combination pharmacologic agent comprising carbidopa, a peripheral aromatic L‑amino acid decarboxylase inhibitor, and levodopa, a dopamine precursor. The combination increases central dopamine synthesis by permitting levodopa to cross the blood–brain barrier while suppressing peripheral decarboxylation, thereby improving motor symptoms in Parkinson disease.
Carbidopa inhibits peripheral decarboxylation of levodopa, enabling greater central availability of levodopa after oral administration. Levodopa is transported into the brain and decarboxylated to dopamine, a neurotransmitter mediating motor control. The net result is improved bradykinesia, rigidity, and tremor in dopamine-responsive parkinsonian syndromes.
Together, the components address both central and peripheral pharmacology: carbidopa reduces peripheral nausea and cardiovascular side effects by limiting dopamine formation outside the brain, while levodopa replenishes dopaminergic signaling centrally. Pharmacokinetics are influenced by food, disease state, and formulation. Levodopa absorption is saturable and variably affected by dietary amino acids; minimal fasting improves absorption, whereas high‑protein meals can reduce peak exposure. Immediate‑release and extended‑release formulations provide different onsets and durations of action.
Primary use is in Parkinson disease with demonstrable levodopa responsiveness. In patients with rigidity, bradykinesia, and tremor, carbidopa/levodopa provides reliable symptomatic control and is considered the standard of therapy for advancing disease or fluctuating motor states.
In patients with motor fluctuations (wearing-off, on–off phenomena) or inadequate response to dopamine agonists, combination therapy with carbidopa/levodopa can restore consistency of benefit. Dosing is individualized and typically begins with low units, such as 25 mg carbidopa/100 mg levodopa per dose, given three times daily for symptom assessment and tolerability. Administration guidelines emphasize timing with meals due to protein interactions; to optimize absorption, patients may take with a small amount of food or on an empty stomach when GI symptoms permit. For advanced disease, extended-release formulations or continuous infusion (Duopa) may be used to maintain steady dopaminergic stimulation.
Oral tablets should be swallowed whole with water; do not crush or chew extended-release forms. For swallowing difficulties, dispersible or orally disintegrating formulations may be available. Regular monitoring for response and dyskinesias is advised, with dose changes guided by a clinician.
Contraindications include hypersensitivity to any component and use with nonselective MAO inhibitors due to risk of hypertensive crisis or serotoninergic adverse interactions.
